Contact Us Form. When you contact Us using the Site’s Contact Us, We collect your name, email address, phone number and the contents of your message that you provide in the contact form you submit to Us via the Ste. We process this information to respond to you, to communicate with you, to manage Our relationship with you, and to keep records of Our communications with you.
 

Your Email Sent to Us. If you send Us an email using the email address displayed on the Site, We collect your email address and the contents of your email messages sent to Us. We process this information to respond to you and otherwise communicate with you, to manage Our relationship with you, and to keep records of Our communications with you.
 

Consultations.
When you book a consultation or purchase a Pure Path wellness service package, you will be redirected to a third-party website (i.e., Practice Better) that hosts Our dedicated booking and payment services, which is not owned or controlled by Us. While Practice Better processes your booking and payment, We will have access to certain personal information you provide, including your name, email address, phone number, other information requested on the booking form, and any additional information you voluntarily provide. However, We do not collect, store or have access to your payment card details or payment method information. This sensitive financial data is handled solely by the third-party payment processor(s) used by Practice Better. The information you provide to Practice Better and payment made using Practice Better’s third-party payment processor(s) is governed by their own respective privacy policies and terms of service. Your Emails and Text Messages. If you are a client of one of Our wellness service packages that includes support by email and text messaging, We collect your email address, telephone number and the contents of your email and/or text messages sent to Us. We process this information based your given consent, to communicate with you, to provide Our wellness coaching and support services to you, to manage Our relationship with you, and to keep records of Our communications with you.
 

At-Home Lab Testing Provider. If you are a client of one of Our wellness service packages that includes at-home lab testing, We may, with your given consent, receive your personal information from Ayubowan (the mobile app) or EquiLife (the web-based application) that may include your purchase history, your answers to any questionnaires that you completed with them, and your lab test results. We collect and process this information based on your given consent and to provide Our wellness coaching and support services to you.  The information you provide to this lab testing provider is governed by its own privacy policies and terms of service. Cookies. The Site uses first-party essential cookies to ensure proper functionality and enhance your browsing experience. These cookies are strictly necessary for the operation of the Site and do not collect any personally identifiable information. Our Web Hosting Provider. Some or all of your personal information collected from the Site is shared with Wix, Our web hosting service provider. When you access or use the Site or purchase of any of Our wellness service packages, your personal information is processed in the United States or in the country where Our third-party services providers maintain their physical or cloud data hosting facilities. IF YOU DO NOT WANT YOUR PERSONAL INFORMATION TO BE COLLECTED, PROCESSED OR TRANSFERRED OUTSIDE YOUR COUNTRY OF RESIDENCE, DO NOT ACCESS THE SITE OR PURCHASE ANY OF OUR SERVICES. By sending Us your personal information, you expressly acknowledge your understanding, consent and agreement to have your personal information processed and stored in the United States by Us and/or in the country where Our applicable third-party service provider maintains its physical or cloud data hoisting facilities.
 

How Long We Retain Your Personal Information

We retain your personal information, including any personal health data, for so long as is reasonably necessary to fulfill Our business purposes as described in this Privacy Policy, to provide Our services to you, and to comply with Our legal obligations. • You control what information you decide to share with Us on social media.

• When you interact with Us on these platforms, you are subject to the platforms’ respective terms of service and privacy policies.

• Any information you share publicly on these platforms (such as comments on Our posts) is visible to others.

• If you send Us a direct message through a social media platform, We receive personal information you choose to share. This could include your name, social media handle, the content of your message, and any other details you voluntarily provide. You should refrain from including any sensitive or confidential information in direct messages to Us.

• We may use information gathered from your social media interactions to respond to your inquiries or comments, improve Our services based on feedback and/or understand Our audience better.

• We do not independently store or maintain records of social media interactions outside of the respective platforms. Retention of messages and interactions is subject to each social media platform’s data retention policies.

• We do not and cannot guarantee the security of data transmitted or stored on social media platforms. Your use of these platforms is at your sole risk.

Your Privacy Rights

Depending on where you reside and the laws applicable to Us, you may have one or more of the below listed privacy rights that you may exercise with respect to Our processing and use of your personal information, subject to Our holding your personal information at the time of your written, verifiable request. Request Submission and Identity Verification Process

We must be able to verify your identity when you exercise any of your available rights. To verify your identity, We may match your information on your request to your personal information We have on file. Part of this process may include Us asking you to provide additional information, such as your name, address, and telephone number.
 

You may designate another person to serve as your authorized agent to act on your behalf to exercise any of your available rights subject to Our being able to verify their identity and authority to act as your authorized agent.

If you are between the ages of 13-16 years old, your parent or legal guardian may exercise any of your available rights subject to Our being able verify their identity and relationship to you.

If you are an adult with a legal guardian or conservator, your legal guardian or conservator may exercise any of your available rights subject to Our being able to verify their identity and authority to act on your behalf.

If you are exercising a right that is the responsibility of another business, We let you know so that you can contact that business directly.

If Our verification process is successful, We will honor the request within a reasonable time of Our receiving your verifiable request (typically within 30 days of Our receipt). We reserve the right to extend the time to respond to you or the requestor acting on your behalf by sending you or the requestor a notice within 30 days of Our receiving your request that states the reason(s) of Our needing additional time.

If We cannot verify your identity, or requestor acting on your behalf, We have the right to deny the request. If there is no reasonable method by which We can verify your identity, or the identity of the requestor acting on your behalf, to the degree of certainty required, We will state this in Our response and explain why We have no reasonable method by which We can verify your or their identity, as the case may be.
