Consultations.
When you book a consultation or purchase a Pure Path wellness service package, you will be redirected to a third-party website (i.e., Practice Better) that hosts Our dedicated booking and payment services, which is not owned or controlled by Us. While Practice Better processes your booking and payment, We will have access to certain personal information you provide, including your name, email address, phone number, other information requested on the booking form, and any additional information you voluntarily provide. However, We do not collect, store or have access to your payment card details or payment method information. This sensitive financial data is handled solely by the third-party payment processor(s) used by Practice Better. The information you provide to Practice Better and payment made using Practice Better’s third-party payment processor(s) is governed by their own respective privacy policies and terms of service. Your Emails and Text Messages. If you are a client of one of Our wellness service packages that includes support by email and text messaging, We collect your email address, telephone number and the contents of your email and/or text messages sent to Us. We process this information based your given consent, to communicate with you, to provide Our wellness coaching and support services to you, to manage Our relationship with you, and to keep records of Our communications with you.
At-Home Lab Testing Provider. If you are a client of one of Our wellness service packages that includes at-home lab testing, We may, with your given consent, receive your personal information from Ayubowan (the mobile app) or EquiLife (the web-based application) that may include your purchase history, your answers to any questionnaires that you completed with them, and your lab test results. We collect and process this information based on your given consent and to provide Our wellness coaching and support services to you. The information you provide to this lab testing provider is governed by its own privacy policies and terms of service.
